Information you might need
If you are working with or know of a young
person who would benefit from any of our courses or services please
complete and send in the request for service form to the e-mail at
the top of the first page.
In order to give young people the best service
we would ask that you complete the form with the young person and
they consent to the information coming to us. If you do not hold
parental responsibility for the young person, please inform the
parents/carers of the young person.
The request for service form is designed to
give us as much information as possible on the young people and
therefore provide the best service for that individual. If the
young person already has a professional assessment (e.g. CAF or
Asset) please complete the first two pages and send to us with the
assessment on the e-mail address at the top of the form.
If the young person does not have an
assessment please fully complete the form and the assessment wheel
and send in on the e-mail address at the top of the page. Please be
advised that all information provided will be shared and discussed
with the young person so that we can make sure that our provision
is right for them.
The Assessment Wheel
The assessment wheel has twelve different
segments that require completing. These areas have been
specifically chosen to give us a full overview of the young persons
situation and will give us an early indication of the best service
to offer them.
Each section has supporting guidance notes
to aid you in your assessment and for you to provide any supporting
information. We will on meeting the young person go over this with
them, so that they are clear on the areas we will be concentrating
on.
The assessment wheel is based on a scoring
system and the guidance gives some indication of what this means,
if you are unclear or want to talk through the assessment please
telephone the numbers below.
When completing the scoring this will
automatically populate the assessment wheel. If we feel that we do
not have sufficient information on the form to deliver a good
service to the young person, we may come back to you for further
information. On the basis of the information supplied and our work
with the young person we may work with other agencies to deliver
the best service.
As we develop our work we will revisit the
assessment at review points to show the progress made with the
young person against agreed objectives. Once our work with the
young person has been completed the exit review and assessment will
be shared with you so that you are informed and involved with the
outcome.
