Help us process your application as quickly as possible by
following our top tips. Any errors in your application may mean
that your card will take longer to process and/or may result in
your application being returned to you.

1. Complete Part A (yellow
section) of the application form using capital letters and
black ink. This can be done by either you or your
parent/guardian.
2. In addition you will need to
provide 1 passport style photograph with your
application. This must:
- be a recent, colour photograph of the head
and shoulders, in sharp focus with the entire face visible;
- show you with a neutral expression with your
mouth closed and eyes open, looking directly at the camera;
- not show you wearing a hat or anything that
covers your face and it must be free of glare, for
example from glasses;
- be taken against a light, plain background;
and
- be taken by a Passport Office approved photo
service for example a photo booth, post office or local
film processing service and be reproduced in standard passport
size.
Please print your name on the back of your photograph.
Do not staple or glue the photograph to the
form.
3. Take your form and photograph to your school
or college and ask your tutor or school office to complete
Part B (green section).
Please also ensure that they sign the back of your
photograph.
4. Complete Part C (pink section)as
appropriate. Please tick ONE of the four options in this
section:
To pay:
- Enclose a cheque or postal order for £50.00 – made payable to
West Sussex County Council, OR
- Pay £50.00 by credit or debit card. Please call 01243 756689
with your card details (we accept JCB, Maestro, Mastercard, Solo,
Visa and VisaDebit). You will be given a payment reference number
to enter in the space provided on the application form.
You are exempt from payment if:

Your 3in1 card will be sent to you in about 4-6 weeks from the
date we receive your completed application form.
When you receive your card please do the
following:
- Check all the details are correct - if they
are not correct, please let us know immediately.
- Keep it safe - contact us straight
away if the card is lost or stolen.
- Don't try to change the card in any way - you
won't be able to use it.
- Ensure that you carry your card at all times
when travelling on the bus.
